Pre-Surgical Optimization Prehab: lower BMI, increase strength, and conditioning

What is Prehab?

You and your surgeon have decided that surgery is necessary to manage your injury or condition, but you were informed that you need to reduce your BMI (body mass index) first. Research has shown having a healthy BMI before surgery not only reduces the risk of infection and complications during the operation but can also improve joint function and reduce pain long-term. This is where Prehab comes in.

The Prehab Program provides a roadmap to help you prepare for surgery and maximize your post-surgical results by pairing you with a dedicated team consisting of a Physical Therapist and a Registered Dietitian. Exercise Specialists may also be utilized. We believe that the road to a healthy weight and lifestyle is a continuous journey, and we will work closely with you and your surgeon to design a program to meet your individual needs, optimize your health, and reduce your BMI in preparation for surgery. Your program will guide you from the initial phases of pre-surgical rehabilitation through your transition to a prescribed exercise routine after your recovery. In some cases, these routines can be continued at your local clinic, under the supervision of an Exercise Specialist.

What’s included in Prehab?

  1. Visits with a licensed Physical Therapist who will conduct an evaluation and establish a plan of care based on your activity restrictions in combination with an assessment of your overall strength, flexibility, and mobility. Treatment may include manual therapy and prescribed exercises aimed at addressing your specific goals and setting you up for success for your upcoming surgery.
  2. Visits with a Registered Dietitian who will perform an assessment and provide recommendations to improve dietary choices, aid in the management of an existing or developing medical condition and create a personalized diet plan based on your specific needs. Your continued sessions will be geared toward making sustainable changes that allow you to be successful in achieving your targeted goals. Working with a Dietitian during the prehab process will not only set you up for success in providing your body with adequate nutrition prior to surgery; it will also help you navigate post-surgical challenges with changes in appetite and energy, changes in nutrient needs through the healing process, and possible changes to your ability to navigate cooking.

Together, we will take the guesswork out of developing a program that takes into account your symptoms, personal limitations, and goals. Medical oversight will be provided by our team of licensed professionals to ensure that your program is safe, effective and aligns with your goals and your surgeon's both pre- and post-surgically.

  • If we are asking you to do something that is hurting too much, let us know! Physical therapy should not be severely painful. It is not common to be so sore that patients are in pain or it affects their daily life. If this ever happens during or after an appointment, let us know and we will adapt your program.

  • We recommend budgeting 40-60 minutes for a typical therapy session. The only exception to this will be your first visit, which can take about 20 minutes longer due to the physical therapy evaluation your therapist will provide. We also recommend showing up about 10-15 minutes early to your first visit. This way, you can complete the paperwork before your visit time and streamline the process.

  • A typical order for physical therapy will ask for 2-3 visits per week for 4-6 weeks. Sometimes the order will specify something different. What generally happens is for the first 2-3 weeks, we recommend 3x per week. This is because it will be the most intensive portion of your treatment. After that, it is common to taper to 2x weekly, based on your level of function and progress.

  • Yes. Physical therapy works based on the science of how the body rebuilds and heals. The biggest killer of progress is inconsistency. We recommend adhering to your prescribed sessions agreed upon in your PT Eval to achieve desired results.
